Some people think hospice is only for the last few days of life when, in fact, patients and families benefit most when they start hospice early. Our team guides them throughout the process in order to bring peace and comfort to everyone involved. LB Hospice offers what patients and families say they want most at the end of life:

Hospice coverage is available through Medicare for people with a life expectancy of six months or less (if the illness runs its normal course). Keep in mind, people do “graduate” from hospice and live longer than six months, so hospice isn’t about giving up hope.

WHEN TO START HOSPICE
Dealing with a life-limiting condition or other health complications can feel overwhelming. If you’re unsure about whether hospice is right for you or your loved one, the best thing to do is start with a call to us right away. We’ll walk you through what you need to know and answer any of your questions or concerns. Some signs that it’s time for hospice:
If the patient has experienced:
- An increase in pain, nausea, breathing distress, or other symptoms
- Repeated hospitalizations or trips to the emergency room
- Failure to “bounce back” after medical setbacks occur
- Increasing assistance needed for walking, eating, bathing, dressing and/or toileting
If the family is:
- Overwhelmed by the burden of physical, financial, emotional, and spiritual concerns
- Unprepared for the complicated medical needs of their loved one
- Afraid to leave their loved on at home alone
- In need of emotional support to cope with the stress of dealing with serious illness and death
Early referral to hospice enhances quality of life and provides many benefits:
- Care plan – Our hospice team will develop a care plan that offers guidance and ensures everyone is on the same page.
- Financial support – Medicare covers the cost of hospice, including medications and medical equipment pertaining to the life-limiting condition.
- Spiritual care – Both patients and families need support emotionally and spiritually. Our chaplains focus on all aspects of a person’s well-being: body, mind, and soul.
- Pain and symptom management – A main goal of hospice is to reduce pain to a level that is acceptable to the patient.
- Prevent hospitalizations – By avoiding trips to the emergency room, hospice helps reduce the stress and financial impact surrounding frequent hospital visits.
We work together as a team to give you or your loved one the best care possible:
- Nursing Care supports the medical care of the patient as needed. A nurse is on call and may be contacted 24 hours a day, every day.
- Social Workers help with the emotional and everyday stresses that often accompany a life threatening illness. They may be called upon to assist with connecting families with community resources to help with financial, legal, or insurance issues.
- Home Health Aides assist in a patient’s personal care.
- Volunteers offer companionship or respite for caregivers on a scheduled or as-needed basis.
- Bereavement Social Workers provide caregivers and members of the community grief support to help them cope with feelings of loss.
- Chaplaincy Services comfort and offer spiritual guidance to patients and families dealing with end-of-life issues.

WE HONOR VETERANS
Today, one in four Americans facing a life-limiting illness is a veteran. These men and women present unique challenges at the end of life due to their military service. Across America, hospice professionals are stepping up to build their skills and accept the mission to serve our nation,s veterans. LB Hospice has heeded the call for our veterans by providing exceptional care with honor and dignity.
Staff use a military history checklist to gather information about the client’s service and ask if they are enrolled with the VA for benefits and services. If not, we ask the client if they need assistance to enroll with the VA.
Staff honor and thank the veteran by presenting a certificate of appreciation for their service and a lapel pin with the branch of service flag and logo of “Honored Veteran.”
STAFF AND VOLUNTEER TRAINING
LB Hospice staff and volunteers undergo training to recognize potential special care needs of veterans due to their time spent in the service. The training identifies the unique cultures within the military and of the different eras when the veterans have served. We share information about the VA as an organization and the benefits veterans can qualify for as part of their service.
Common language and experience can provide camaraderie that is supportive for all involved. Volunteers provide relief for family/caregivers, assist in reminiscing, and gather life stories with a potential for recording them for the family or the Veteran History Project. Volunteers give veterans companionship and may also take part in the recognition/pinning ceremonies.
COMMITMENT TO VETERANS
LB Hospice is working to build relationships and networks with local and regional organizations that serve veterans to increase access to care. Our goal is to learn and to provide education in the communities we serve. LB Hospice offers:
- Advanced Illness Management and End of Life Care with honor and dignity
- Help at any point in an advanced illness and for those with a limited life expectancy
- Preventing, relieving and managing pain and other symptoms
- Focusing on veteran’s choices, goals, desires and quality of life
- Providing social, spiritual and emotional support to the veteran and their family
- Availability of 24 hour on-call support
- Provide an individualized plan of care that meets the needs of the veteran and their family
A Veteran can receive care wherever they live, such as their home, foster home, caregiver’s home, assisted living facility, group home, community nursing home or state Veteran’s Home.
Care is provided by an interdisciplinary team that includes the veteran’s physician, LB Hospice nurses, and as needed, home health aide, social worker, chaplain, and trained volunteers.
“We Honor Veterans” is a project of the National Hospice and Palliative Care Organization in performance of a contract with the United States Department of Veterans Affairs.

To qualify for hospice care, a patient must:
- Obtain the diagnosis of a serious illness with a prognosis not expected to be longer than six months if the disease takes its normal course
- Acquire physician approval
- Desire care for pain and symptom control with a focus on quality of life
- Live within the service area of LB Hospice
Please understand that the six-month prognosis is merely a guideline. Hospice re-evaluates patients periodically and recertifies them for hospice coverage as long as they continue to meet the guidelines.
